Long-term care insurance: Do you actually need it?
Most people will need long-term care eventually. Medicare doesn't cover it. Here's how to think about insuring vs. self-funding, plus the hybrid alternative that's quietly taken over the market.
About 70% of Americans 65 and older will need some form of long-term care during their lifetime, per the US Department of Health and Human Services. For most, it's a brief assist after a hospital stay; for a meaningful minority, it's multiple years of expensive custodial care. Medicare covers very little of it.
There are three ways to pay for the expensive scenario: long-term care insurance, self-funding from a portfolio, or Medicaid after spending down to near-zero assets. Each has trade-offs.
The actual cost of care (2023 medians)
From the Genworth Cost of Care Survey (2023, the most recent comprehensive US data):

Costs in high-COL states (CA, NY, MA, NJ) often run 40–60% above these medians. Costs in low-COL states (TX, GA, LA) often run 20–30% below.
Length of care
Among those who need any LTC, the average duration is roughly 3 years; women average 3.7, men 2.2 (HHS data). About 20% will need 5+ years of care. A 5-year stay in an average nursing home is roughly $525,000+ in today's dollars, and costs are inflating at 3–5%/year.
What Medicare and Medicaid actually cover
Medicare covers up to 100 days of skilled nursing facility care when:
- You had a qualifying inpatient hospital stay of 3+ days, AND
- You're admitted to a skilled nursing facility within 30 days, AND
- You require skilled care (not just custodial assistance).
Days 1–20: Medicare pays 100%. Days 21–100: you pay $204/day coinsurance (2024). Day 101+: you pay 100%. Medicare does NOT pay for custodial care, assisted living, or home-health aides for daily-living assistance.
Medicaid is the de-facto US long-term care insurance for people who can't afford to pay otherwise. It covers nursing-home care (and increasingly, some home-care alternatives) — but you must "spend down" to ~$2,000 in non-exempt assets to qualify. Income limits also apply. Some assets (primary home up to a value cap, one car, prepaid burial) are exempt. The state can recover costs from your estate after death (Medicaid Estate Recovery).
Traditional LTC insurance
A standalone LTC policy pays a daily benefit (typically $150–$300/day) when you can't perform 2+ of the 6 activities of daily living, or have cognitive impairment.
The premium reality
For a healthy buyer at 55 in 2024 (American Association for Long-Term Care Insurance):
- Single male, $165,000 of benefits: ~$1,700/year
- Single female, $165,000 of benefits: ~$2,700/year (women's premiums are higher because women statistically need longer care)
- Couple (both 55), $165K each: ~$3,750/year combined
- 3% annual inflation rider: typically adds 50%+ to premiums but is essential — without inflation protection, a 25-year-old policy is materially eroded by the time you need it
At 65, premiums roughly double. After 70, most insurers won't write a new policy at any price. Health rating is meaningful — common conditions (diabetes, MS, mild cognitive impairment) often result in higher premium tiers or outright decline.
The "rate-hike" risk
Traditional LTC insurance has a troubled industry history. Companies sold policies in the 1990s–2000s on assumptions about how often claims would happen and how long people would live — assumptions that proved badly wrong. Many carriers exited the market; survivors have repeatedly raised premiums on existing policyholders by 20–80% over policy lifetimes (with state regulator approval).
If you buy a traditional LTC policy, plan for premium increases. Many buyers regret locking in fixed-income retirement budgets that the policy then breaks.
Hybrid policies (the modern alternative)
A hybrid policy is a permanent life insurance contract (or annuity) with a long-term-care rider. If you need LTC, the policy accelerates the death benefit (or pays a multiple of premium) to fund care. If you never need LTC, your beneficiaries get the death benefit.
How a typical hybrid works
- Premium structure: single-pay (~$100K lump sum) or 10-pay (10 annual payments). Less common: lifetime-pay.
- If LTC is needed: typically pays a monthly benefit equal to 2–4% of the face amount, up to a cap (e.g., 200% of face).
- If LTC is never needed: heirs receive the face amount as life insurance proceeds.
- If you change your mind: most hybrids include a return-of-premium feature (~80–100% refund).
Why hybrids took over
Hybrid policies have grown from ~20% to ~75% of new LTC-style coverage sales in the past decade. The pitch: no "use it or lose it" risk. If you pay $100K in premium and never need LTC, you (or your heirs) get a substantial death benefit back. With traditional LTC, the same $100K of premium has no return if you stay healthy.
Downside: hybrids are more expensive upfront. The same monthly LTC benefit costs ~2–3× the cumulative premium of a traditional standalone policy.
The self-funding option
If your investable net worth is large enough, self-funding makes sense. A 3-year care event in average circumstances costs roughly $200K–$350K in today's dollars. A 5-year event in a high-COL nursing home can run $500K–$700K+.
Rough net-worth thresholds for self-funding to be reasonable:
- Single: $2M+ in investable assets (not counting primary home).
- Couple: $3M+ in investable assets.
Below those thresholds, a major LTC event can deplete a retirement portfolio fast, leaving the surviving spouse with insufficient assets. Above them, self-funding sidesteps premium-hike risk and insurer-default risk.
The Medicaid option
For people whose assets are already low (or who plan to spend down), Medicaid is the default safety net. Not really a planning strategy for most people — you must spend nearly all non-exempt assets first, you have limited control over which facility accepts you, and the state can recover costs from your estate.
"Medicaid planning" via asset transfers requires a 5-year lookback period and is increasingly restricted. Talk to an elder-law attorney specifically — generalized advice here can backfire.

What to look for in a policy
- Benefit triggers: Loss of 2+ ADLs (out of 6) OR cognitive impairment. Standard across major carriers.
- Daily benefit amount: Match to nursing-home costs in your area (or area where you plan to retire). $200–$300/day for average areas; higher in CA/NY/MA.
- Benefit period: 3 years is the most common. 5-year extends to ~95% of LTC events. Lifetime is rare and expensive.
- Elimination period (waiting period): 30, 60, or 90 days. Longer = lower premium. 90 days is standard and matches what people can self-fund.
- Inflation rider: 3% compound minimum. Essential — without it, the policy erodes over decades.
- Carrier financial strength: Look for A+ or AA rating (A.M. Best / S&P). Avoid mid-tier or smaller carriers.
- Spousal/partner discount: 15–30% discount when both partners buy from the same carrier. Always ask.
How to actually shop

The bottom line
The honest answer: most people in the middle-class wealth band ($500K–$2M of investable assets) should seriously consider some form of LTC coverage. Hybrid policies have largely replaced traditional standalone LTC in the new-sale market because they avoid the "use it or lose it" problem and have more stable premiums.
High net-worth households can self-fund. Lower-net-worth households face Medicaid as the default and can't really afford the premiums anyway.
Regardless of which path you choose, the planning conversation should happen at 55–60, not 70. Underwriting tightens with age, premiums rise sharply, and after 70 most insurers won't write a new policy at any price.
