The six-step playbook

  1. Confirm you're HDHP-eligible.Your health plan's annual deductible must be ≥ $1,700 (self-only) or $3,400(family) in 2026. Out-of-pocket max ≤ $8,500 / $17,000.
  2. Max the contribution. Target $4,400 or $8,750 in 2026, plus the $1,000 catch-up at 55+. Use payroll deduction to capture FICA savings.
  3. Move money out of cash, into investments.Most HSA providers default you into a cash account paying near-zero interest. Allocate the bulk of the balance to a low-cost total-market ETF or target-date fund once you cross the provider's investment threshold ($1,000–$2,000 commonly).
  4. Pay current medical costs out of pocket, if your cash flow allows. Co-pays, prescriptions, dental cleanings — pay from checking, leave the HSA invested.
  5. Save every medical receipt. A photo or PDF in a labeled folder is enough. You can reimburse yourself decades later, tax-free, as long as the expense happened after your HSA was open.
  6. Roll the employer HSA to a low-fee HSA once a year. Free trustee-to-trustee transfers; no tax impact; no limit on these between-HSA transfers.

The two valid exit strategies

You don't have to commit to one path at the start. The HSA gives you optionality decades in advance.

  • Strategy A: Tax-free medical bucket. Save receipts for life. In retirement, reimburse yourself for past medical expenses, tax-free, in whatever amounts you need. Tax-free cash whenever you want it, indefinitely.
  • Strategy B: Stealth Traditional IRA. After 65, treat the HSA like a Traditional IRA — withdraw for any reason, pay ordinary income tax. No 20% penalty. No required minimum distributions during your lifetime (unlike Traditional IRAs).
  • Most people mix: reimburse past medical expenses tax-free first, then dip into the remaining balance taxably for everything else.

Common mistakes

  • Leaving the balance in cash. The biggest one. A $50,000 HSA balance earning 0.5% loses to inflation every year. Once you have the cash for medical surprises (3–6 months), invest the rest.
  • Forgetting to save receipts. Without receipts, you can't reimburse past medical expenses tax-free decades later. Scan as you go; a labeled cloud folder is fine.
  • Using a bad employer HSA. If yours charges $3/month or has only mutual funds with 0.50%+ expense ratios, roll the balance over annually to a low-fee provider.
  • Contributing while not HDHP-eligible. If your plan changes mid-year, your contribution limit pro-rates. The IRS hits you with a 6% excise tax on over-contributions until you correct them.
  • Pairing with a disqualifying general-purpose FSA. A standard healthcare FSA blocks HSA contributions. If both are offered, decline the FSA or choose a Limited-Purpose FSA (LPFSA) instead.
